About the JobCook Children's Health Care System is seeking a skilled Clinical Pharmacy Specialist to join our team in Fort Worth, Texas.This is a unique opportunity to work with a world-class healthcare organization and contribute to the care of patients with renal transplant or nephrological conditions.The ideal candidate will have a strong background in clinical pharmacy practice, excellent communication skills, and the ability to work effectively in a fast-paced environment.Responsibilities

• Develop and implement comprehensive medication regimens for patients with renal transplant or nephrological diseases
• Provide medication counseling and education to patients and their caregivers
• Monitor patient response to therapy and adjust medication regimens as needed
• Collaborate with healthcare providers to optimize patient outcomes
• Participate in transplant evaluation and post-transplant care
• Manage medication-related adverse events and drug interactionsRequirements
  • PharmD from an ACPE accredited school of pharmacy
  • Completion of a PGY1 and applicable PGY2 specialty residency or a minimum of 5 years of clinical experience
  • TX Pharmacist license or eligible
  • Preceptor license required (must be obtained within 90 days)
  • CPR - BLS required
